Try in the following order:
1. Run Device Analysis Test in Library and Tape Tools

2. Run Read/Write Test in Library and Tape Tools
.
3. Run Media Validation Test in Library and Tape Tools
.
4. Upgrade to latest firmware
.
5. Check back-up logs
.
6. Check you are using a supported configuration and correct media
.
7. Check if verify has failed
.
8. Check if the scheduled back-up has failed
.
9. Check for media damage
.
10. Check that the drive has been cleaned
.
11. Check for back-up software issues

12. Check you can format a tape
.

Run the Device Analysis Test in HP Library and Tape Tools
(L&TT). Follow any troubleshooting given with the test
results.
Ensure the latest firmware is loaded.
NOTE:
Due to issues in development Library and Tape
Tools cannot be reliably used to diagnose potential
issues with the DAT 40 Hotplug drive (Q1546A
only). This problem is currently being investigated
and will be corrected in the next revision of Library
and Tape tools. This note will be updated once the
problem has been corrected but until then do not
use Library and Tape Tools to diagnose issues with
this product. This problem only relates to the
