  This indicates that all phase protection has been compromised or lost. It could also mean the SPD rated voltage is less than the system voltage, or there's an extended Temporary Overvoltage (TOV), or a significant surge event. The suggested solution is to replace the SPD, replace the SPD with the correct voltage model, and check the electrical system for TOV sources; correct, and replace the SPD.
