3.9 Making an ECochG recording
An ECochG (Electrocochleography) is a measure of the response of the cochlear (summating potential (SP),
action potential (AP), and cochlear microphonic (CM)). ECochG is used in for multiple purposes i.e. in the
diagnosis of Meniere’s disease, Perilymph Fistula, and sudden deafness.
It is recommended that Tiptrodes, TMtrodes or Transtympanic electrodes be used to measure the
electrocochleogram. While transtympanic electrodes will result in the most robust response, it may not be
feasible in many clinics.
3.9.1 ECochG electrode montages
TipTrode
Vertical montage
• (+) Active electrode (tiptrode) is placed on the high forehead or Cz.
• (-) Reference electrode is placed in the ear canal (gold foil electrode).
• Common electrode is placed on the low forehead.
TMtrode
• (+) Active electrode is placed on the tympanic membrane
• (-) Reference electrode is placed on non-test (mastoid/earlobe)
• Common electrode is placed on the forehead
• Switch the reference and active electrodes when changing test ear
