SELECT DATE FORMAT
Select desired date format from the list; yyyy-MM-dd, MM-dd-yyyy, etc.
SELECT TIME FORMAT
Select either HH:mm – HH:mm:ss – or – h:mm a
CONSECUTIVE# IS RANDOM
If selected the consecutive # is generated randomly, rather than sequentially.
